Assertive Community Treatment (ACT) is an evidence-based model of care for adults with serious mental illness that provides comprehensive, community-based services, including behavioral health and substance use treatment, healthcare coordination, criminal justice support, and other individualized supports.
Each of our three ACT teams works closely with participants and their families to reduce reliance on more restrictive levels of care and enhance quality of life within the community. Services are tailored to each individual's unique needs, strengths, and recovery goals, with treatment driven by participant choice and engagement.
Our multidisciplinary ACT teams provide psychiatric, nursing, clinical, forensic, vocational, and benefits expertise to deliver integrated, person-centered care. Through ongoing support and collaboration, ACT promotes recovery, community integration, and increased independence.
